Florida Agency for Health Care Administration (AHCA)
The AHCA serves as the chief health policy and planning entity in Florida.
The agency is responsible for administering Medicaid, licensing and regulating
health facilities and providing information to residents about the quality
of health care they receive in the state. The website contains performance
and outcome data for health care facilities in Florida as well as information

Community Health Needs Assessment
From July 2013 through September 2014, the St. Johns County Health Leadership
Council, co-chaired by Flagler Hospital's Director of Community Health
Improvement, completed a comprehensive, county-wide Community Health Assessment
using a nationally recognized approach call MAPP (Mobilizing for Action
through Planning and Partnerships). The Council then used the data collected
in the County Health Assessment to develop a Community Health Improvement Plan.
