St. Johns County Health Leadership Council Working on County's Fourth Communtiy Health Assessment

The St. Johns County Health Leadership Council is working to complete its fourth comprehensive Community Health Assessment and Community Health Improvement Plan, which will be published in August of 2014.

An important component of the Community Health Assessment is a survey that is being conducted during February and March. The survey asks St. Johns residents to share their opinions about their health and the quality of life in here in the county. Your feedback is important, so if you are asked to fill out a How Healthy is St. Johns County? survey, please take a few minutes to complete one. Your input will help make St. Johns County a better place to live. Paper surveys will be offered at numerous locations throughout the county, and the survey is also available online at:

2014 Community Health Assessment Survey

Although St. Johns County remains the healthiest county in Florida, according to the Robert Wood Johnson/University of Wisconsin’s Population Health Institute’s 2013 County Health Rankings Report, the information collected in the survey will be used to develop a Community Health Improvement Plan, which will address opportunities for improvement identified during the assessment process.
