About Flagler Surgery Center
Flagler Hospital is pleased to announce that we have acquired St. Augustine Surgery Center (SASC) located at 180 Southpark Boulevard in St. Augustine. The surgery center is currently under construction to meet certain state licensure requirements and also incorporate design enhancements. Flagler Hospital plans to utilize the outpatient surgery center at its main location during construction, until the freestanding center reopens as Flagler Surgery Center in February.
All surgeries scheduled until early February will take place at Flagler Hospital's outpatient surgery center located on our main campus. Have you been scheduled for surgery and were provided a yellow sheet of instructions? Please click on the link below to complete your pre-surgery packet. PLEASE NOTE: IF YOU DID NOT RECEIVE A BRIGHT YELLOW INFORMATION SHEET, PLEASE CALL 904-823-1447 TO CONFIRM YOUR SURGERY BEFORE COMPLETING YOUR PRE-SURGERY PACKET BELOW.

Your care will be provided by a highly trained professional staff that has been carefully selected for their technical skills and caring interpersonal styles. We pride ourselves on both our professionalism and compassion.
The main number of the center is 904-823-1447. We are open Monday through Thursday 6:30 a.m. to 5:00 p.m. On Friday, the business office is open 8:00 a.m. to 4:30 p.m.
We strive to make your visit with us as pleasant and comfortable as possible. When the physician’s office schedules your procedure with us, we contact your insurance company to determine your benefits, and we will make every attempt to inform you of any money which will be due on the day of your procedure. In order to better accommodate our patients, we gladly accept VISA and MasterCard. You should receive a phone call from a financial counselor and a nurse prior to your procedure.
