What is Flagler Hospital Follow My Health patient Portal?
The patient portal is a secure, online patient portal designed to help you access and manage your health information when it is most convenient for you. The portal is powered by FollowMyHealth.

Is there a fee to use the portal?
The FollowMyHealth patient portal is a free service provided by Flagler Hospital and First Coast Health Alliance.

How will I know when something has been added to my portal?
Log on at any time to review your information. You may also decide to receive email and/or text notifications when information is revised or added to your account. Select "My Account," then "Preferences" and customize your notification settings to meet your needs.

Do I need any special equipment to use the portal?
The Flagler Hospital patient portal, powered by FollowMyHealth, can be accessed from any computer, tablet or mobile device with an Internet connection. It is supported by the following web browsers: Internet Explorer 8.0 or higher; Mozilla Firefox ; Google Chrome; Safari. The FollowMyHealth application is also available for iOS and Android devices.

ENROLLING

How do I set up a portal account?
Sign-up is simple! You can sign up on your own by clicking creating a portal account, or receive an invitation to connect when you provide your email address to us upon admission at the hospital. Once you create your portal account, request to connect to the Flagler Hospital portal! Once we verify your identity, you will be connected to your health record at Flagler Hospital.

Once you sign up, watch for an email from noreply@followmyhealth.com within a few business days. Click the registration link to complete the process.

What if I don't receive the invitation email?
To ensure that the MyHealth invitation email reaches you, please check your "spam folder" and add noreply@followmyhealth.com as an approved sender.

Here is how to add an approved sender to popular email domains:

Yahoo

  1. Open Yahoo email account
  2. Click Options
  3. Click Settings
  4. Click Filters
  5. Click Add
  6. Enter in the email address: noreply@followmyhealth.com
  7. Use the drop down "Then deliver the email to the following folder": Select Inbox
  8. Click Save

Gmail

  1. Open Gmail account
  2. Select contacts from the options on the left side of the Gmail Inbox
  3. Select Create Contact on the top menu
  4. Enter in the email address: noreply@followmyhealth.com
  5. Click Save

AOL, Comcast, Frontier, Centurylink, Embarqmail, Qwest

  1. Open email account
  2. Click on Contacts (or address book)
  3. Click New Contact
  4. Enter in the email address: noreply@followmyhealth.com
  5. Click Add Contact

Hotmail and Web Outlook.com

  1. Open email account
  2. Click on Settings (small gear icon at top right)
  3. Click Safe and Blocked Senders
  4. Enter in the email address: noreply@followmyhealth.com
  5. Click Save

Windows Live and Outlook Application

  1. Open email account
  2. Click on Home tab in the ribbon
  3. Click on Junk button
  4. Select Safety options from drop-down menu
  5. Click Add
  6. Enter in the email address: noreply@followmyhealth.com
  7. Click OK

MSN

  1. Open MSN email account
  2. Select Settings: Email | Junk e-mail (bottom left, just above Calendar)
  3. From the E-mail settings screen, select Junk E-mail Guard
  4. Select Safe List
  5. Enter in the email address: noreply@followmyhealth.com
  6. Select Add

What is my username? My password?
You determine your own username and password when you create your portal account online.

After I set up my account, when and how do I log in?
Log on whenever you want to review changes, make updates, request appointments and more. Bookmark the site to make it convenient to visit on a regular basis. A link the portal is also available at Flaglerhospital.org/followmyhealth. You may also decide to receive email and/or text notifications when information is revised or added to your account. Select "My Account," then "Preferences" and customize your notification settings to meet your needs.

ACCESS

Can I give others access to my information?
Only you – and those you authorize – will be able to see or gain access to the information in your portal. You can give others proxy approval by completing the proxy approval steps available once you log into your account.

PASSWORD / LOG-IN ISSUES

What if I forget my password?
If you don't remember your password, choose FMH Secure Login, enter your username and select "Reset." You will then receive an email with instructions about how to complete this process. If you originally signed up using another account (Facebook, Google, Yahoo or LiveID), select the appropriate icon and follow the instructions.

Please note: If you request three password resets in a 24-hour period and do not reset the password, this option will be locked for 24 hours.

What if I have trouble logging in or using the portal?
Call the FollowMyHealth help line at 888-670-9775 or send an email to support@followmyhealth.com. Support is available Monday – Friday from 8am – 8pm.

CONFIDENTIALITY AND PRIVACY

Will my information in the portal be secure?
Yes, your personal and medical information in the MyHealth portal will be kept confidential. MyHealth uses a secure, encrypted connection that meets the highest industry standards. All health data is stored on a secure server.

Only you – and those you authorize – will be able to see or gain access to the information in your portal. We will not share your personal information with any outside party not affiliated with our organization.
